Thank you for completing the New Patient Registration Form. We’re excited to welcome you and your pet to our family. Our team will review your information and reach out to collect the $78.50 deposit required for new clients, which covers the exam fee. If your pet has been seen at another facility prior to your visit, please have the name of the facility ready so we can request records be transferred and update our files.
